Florida accepts the Uniform Application to Register Securities (Form U1), and allows for electronic signature of forms.

A Consent to Service of Process (Form U2)
SEC Form 1-A with exhibits
Detailed itemization of all offering expenses
Corporate resolution
Incorporation documents and exhibits
Options and warrants policy
Applicant escrow agreement of offering proceeds (if applicable)
Debt securities trust indenture
Florida collects a fixed fee of $1000. Payments are submitted through a proprietary licensing system via credit card or ACH payment. Florida, by rule, does not accept paper checks.
